Program Guidelines:

• Membership begins on date of sign-up and covers one year.
• Membership automatically renews on the sign-up date each year. If a member wishes to discontinue their membership, they may do so at any time by calling the office. Members will be informed of renewal one month in advance.
• Membership fee is due the day of joining the program and is non-refundable, even if member does not return for appointments during the year.
• All fees for dental services performed during each appointment must be paid in full by the end of the appointment. Members of T – Town Smiles Dental Savings Plan may be eligible for in-house financing, if available.
• Preventive treatment (2 cleanings, 2 exams, and x-rays) included in each year of membership do not “roll over” to the next year.
• Any children listed on plan must live in household or be a full time student.
• Rates are subject to change annually; all members will be informed of any future changes before renewal.

T – Town Smiles Dental Savings Plan is not a dental insurance plan, therefore certain limitations apply:

• Membership discounts cannot be used in conjunction with any other special or discount. Insurance benefits cannot be combined with this program. Discounts under this savings plan shall not apply to any treatment started prior to enrollment or after membership expires.
• Membership discounts cannot be combined with dental care covered under automobile insurance, medical insurance, or worker’s compensation.
• This membership only covers treatment provided at T – Town Smiles; if a patient is referred to a specialist, discounts offered to members at T – Town Smiles will not apply at the specialist’s office.
• Membership discounts do not apply to consumable items such as electric toothbrushes, prescription toothpaste, etc
• Membership discounts do not apply to orthodontic treatment (Invisalign, ClearCorrect, etc)
